Epidural Steroid Injections for Herniated Disc Pain

An epidural steroid injection is one of the most commonly used nonsurgical treatments for herniated disc pain that has not responded adequately to activity modification, medication, and physical therapy. Understanding what the injection does — and what it does not do — helps patients make informed decisions about their care.

What an Epidural Steroid Injection Does

The American Academy of Orthopaedic Surgeons notes that an epidural steroid injection "may provide short-term pain relief by reducing inflammation."

The injection delivers corticosteroid medication into the epidural space — the area surrounding the spinal cord and nerve roots. The goal is to reduce the inflammatory response around the compressed nerve root, which is a primary driver of radiating leg pain (radiculopathy). By reducing inflammation, the injection can decrease pain and improve function enough to allow meaningful participation in physical rehabilitation.

What an Epidural Steroid Injection Does Not Do

It is equally important to understand the limitations:

  • It does not repair the disc. The herniated material remains in place. The injection addresses the inflammatory response, not the structural problem.
  • Relief may be temporary. Some patients experience significant improvement for weeks to months. Others have a partial or short-lived response. A small number do not respond at all.
  • It is not a cure. Even when the injection provides excellent relief, the underlying disc pathology persists. Recurrence of symptoms is possible.

Who Is an Appropriate Candidate

Epidural steroid injections are most likely to be helpful when:

  • The primary complaint is radiating leg pain (radiculopathy) rather than isolated back pain
  • Imaging confirms nerve root compression at a level that corresponds to the patient's symptoms
  • Symptoms have not resolved with initial conservative measures
  • The patient does not have contraindications to corticosteroid administration

Injections are generally less effective for isolated axial back pain without a radicular component.

How the Injection Is Performed

Epidural steroid injections are performed under fluoroscopic (X-ray) or CT guidance to ensure accurate needle placement. The approach — interlaminar, transforaminal, or caudal — is selected based on the location of the pathology and the clinical goals. The procedure is typically performed on an outpatient basis.

How the Response Guides Subsequent Treatment

The response to an epidural injection provides useful clinical information. A patient who experiences significant but temporary relief may be a candidate for a repeat injection or for surgery if symptoms recur and become disabling. A patient who experiences no relief may need further evaluation to confirm the diagnosis or consider alternative treatment approaches.
