What These Treatments Are

Orthobiologic and peptide-based treatments are nonsurgical options considered for selected musculoskeletal conditions. They are not a single category of treatment — platelet-rich plasma (PRP), cellular products, and peptide therapies are distinct treatment types with different mechanisms, evidence bases, and regulatory considerations. They must not be treated as interchangeable.

These treatments are offered only after a thorough individual medical evaluation. Dr. Blythe will explain the available evidence, limitations, alternatives, regulatory status, expected cost, and whether a structural treatment may be more appropriate for your condition.

Patient Evaluation

Determining whether an orthobiologic or peptide-based treatment is medically reasonable for your condition requires a comprehensive evaluation. This may include:

  • Review of imaging (MRI, CT, X-ray)
  • Prior treatment response
  • Medication and medical-risk review
  • Identification of the actual pain generator

Conditions Considered

Stem-Cell and Cellular Treatments

Limitations

These treatments are not appropriate for every patient or every diagnosis. They do not mechanically stabilize an unstable joint, decompress a compressed nerve, repair every complete tendon tear, replace severely damaged joint surfaces, or correct every structural problem requiring surgery.

Regulatory and Evidence Disclosure

The evidence, regulatory status, and accepted indications differ among specific orthobiologic, cellular, and peptide-based treatments. Some treatments or uses may be investigational or off-label. What are orthobiologic treatments?
Orthobiologic treatments use biological substances — derived from the patient's own blood, bone marrow, or from processed donor tissue — to support healing in joints, tendons, and soft tissue. Examples include platelet-rich plasma (PRP), bone marrow aspirate concentrate (BMAC), and amniotic or umbilical cord-derived products. These are adjunct treatments evaluated on a case-by-case basis and are not cures for arthritis or structural damage.
Are stem-cell treatments FDA-approved for orthopedic conditions?
Most stem-cell and cellular orthobiologic products are not FDA-approved for specific orthopedic indications. PRP is prepared from the patient's own blood and is not subject to the same regulatory pathway as drugs. Amniotic and umbilical cord products are regulated as human cells, tissues, and cellular and tissue-based products (HCT/Ps) under 21 CFR Part 1271. Who is a candidate for orthobiologic treatment?
Candidacy is determined after individual evaluation, review of imaging, and discussion of the diagnosis and treatment goals. Orthobiologics may be considered for patients with early to moderate joint degeneration, tendon injuries, or soft-tissue conditions who have not responded adequately to conservative treatment and are not yet candidates for surgery.
Can orthobiologic injections replace surgery?
No. Orthobiologic injections do not repair torn tendons, remove disc herniations, decompress nerves, or replace arthritic joints. They may reduce symptoms or improve the local biologic environment in selected cases, but they do not correct structural problems that require surgical repair.
