Print all pages, fill them out by hand, and bring the completed packet to your appointment.
Bring to your visit
Hand the completed forms to the front desk when you arrive. Also bring a photo ID and your insurance card.
New Patient Paperwork
The packet includes a patient information form, medical history, current medications, and consent for treatment. All pages must be completed.

What to bring to your first visit
- Completed new patient paperwork (this packet)
- Photo ID
- Insurance card
- MRI or CT imaging — on disc, USB, or accessible through a patient portal
- X-rays if available (standing films are especially helpful for spine conditions)
- Operative reports from any prior spine, shoulder, or knee surgeries
- List of current medications and prior treatments (injections, physical therapy)
- Workers compensation claim information if applicable
